ACCEPTABLE FORMS OF IDENTIFICATION FOR THE PS FORM 1583
The following
information is printed in Box 8 of the PS Form 1583. Social Security cards,
credit cards, and birth certificates are specifically listed as unacceptable
forms of identification for the U.S. Postal Service.
One form of identification must include
a photograph of the applicant.
Acceptable forms of identification are
limited to the following:

A current, valid
driver's license

A current, valid state
non-driver's identification card
·
Uniformed Services
(Formerly knows as the Armed Forces) ID
card
·
Government agency I D
card
·
University ID card
·
An ID card from a
recognized corporation
·
Passport
·
Alien registration card
·
Certificate of
naturalization
·
Current lease, mortgage
or Deed of Trust
·
Voter registration card
·
Vehicle registration
card
·
Home insurance policy
·
Vehicle insurance policy
